Why Lyrfigtu's FGFR2 Selectivity Matters More Than Its 46% Response Rate

FDA cleared lirafugratinib (Lyrfigtu) for FGFR2-altered cholangiocarcinoma after ReFocus posted a 46% response rate. The clinical argument is FGFR1 sparing. The cost is months of mandatory retinal monitoring.

On September 23, 2026, the FDA approved lirafugratinib (Lyrfigtu, Elevar Therapeutics) for adults with previously treated, unresectable, locally advanced or metastatic cholangiocarcinoma harboring an FGFR2 gene fusion or rearrangement. Reviewed under Real-Time Oncology Review and an Assessment Aid, the oral kinase inhibitor is dosed at 70 mg once daily.

In 116 FGFR inhibitor-naive patients on the pivotal ReFocus trial (NCT04526106) who had progressed on prior systemic therapy, Elevar reported a confirmed objective response rate of 46% (95% CI: 36% to 55%) per independent review. Median duration of response reached 11.8 months (95% CI: 7.5 to 13.0), while median progression-free survival reached 11.3 months (95% CI: 9.2 to 14.8). Those metrics sit slightly above established pan-FGFR labels. Yet response rate alone does not explain why the drug matters. The sharper story is chemical selectivity.

A covalent FGFR2 bet over pan-FGFR inhibition

Lirafugratinib originated as RLY-4008 at Relay Therapeutics, designed on the company's Dynamo computational platform. Cancer Discovery published its defining profile: a covalent small molecule selective for FGFR2, showing more than 250-fold selectivity over FGFR1, 80-fold over FGFR3, and 5,000-fold over FGFR4. The drug also retained preclinical activity against acquired gatekeeper and hinge mutations that defeat first-generation agents, including FGFR2 V564F/L/I and N549K/D.

That molecular profile forms the clinical argument. Pemigatinib (Incyte's Pemazyre) demonstrated a final FIGHT-202 objective response rate of 37.0%, median response duration of 9.1 months, and median PFS of 7.0 months. Futibatinib (Taiho's Lytgobi) in the FOENIX-CCA2 trial reached a 42% ORR, median response duration of 9.7 months, and median PFS of 9.0 months. Cross-trial comparisons carry caveats, but Lyrfigtu leads the class on paper. Even so, few oncologists would switch based on single-digit response gains alone.

Escaping the FGFR1 hyperphosphatemia treadmill

The practical differentiator is phosphate handling. First-generation pan-FGFR inhibitors hit FGFR1, disrupting FGF23-mediated renal phosphate excretion and causing dose-limiting hyperphosphatemia. In FIGHT-202, any-grade hyperphosphatemia hit 58.5% of patients. In FOENIX-CCA2, grade 3 hyperphosphatemia reached 30%. By sparing FGFR1, lirafugratinib reduced hyperphosphatemia incidence to 21%. Only one patient (0.3%) required a dose interruption for phosphate elevation, and none discontinued permanently.

For oncology clinics, this provides immediate operational relief. Routine phosphate binders, dietary restrictions, and frequent mid-cycle serum monitoring have burdened the FGFR workflow since pemigatinib entered practice. A daily oral therapy that avoids that metabolic burden simplifies management, provided it does not introduce an equal liability elsewhere.

The ocular surveillance trade-off

Lirafugratinib introduces its own clinical burden. Across 385 patients in the broader safety cohort, retinal pigment epithelial detachment (RPED) occurred in 31%, with 1.8% experiencing grade 3 events. Median time to first onset was 57 days. RPED triggered dose interruptions in 15% of patients and dose reductions in 10%. Blurred vision occurred in 18%, dry eye in 38%, and corneal toxicity in 11%.

The FDA label consequently mandates comprehensive ophthalmological exams, including optical coherence tomography (OCT) of the macula, at baseline, every 2 months for the first 13 months, and every 4 months thereafter. Epithelial toxicities were also widespread: nail toxicity, hand-foot syndrome, stomatitis, and alopecia each occurred in more than 20% of patients, with serious adverse reactions reaching 32%. Oncologists are effectively trading a metabolic toxicity managed with binders and labs for an on-target ocular toxicity requiring specialized ophthalmology coordination and frequent retinal imaging.

Commercial reality in a rare patient population

Cholangiocarcinoma is diagnosed in roughly 8,000 Americans annually. FGFR2 fusions or rearrangements occur in about 10% to 15% of intrahepatic cases, creating a narrow commercial target in the second-line setting. Relay licensed global rights to Elevar, a subsidiary of South Korea's HLB Co., in December 2024 to focus internal capital on its clinical PI3K-alpha inhibitor, RLY-2608. Relay remains eligible for up to $75 million in upfront and regulatory milestones, $425 million in commercial milestones, and tiered royalties in the low teens.

Elevar must now launch into a niche already contested by two established pan-FGFR drugs. Lyrfigtu delivers the cleanest FGFR2 selectivity to reach market, paired with broader coverage against known resistance mutations. Its commercial adoption will depend less on whether its 46% response rate looks superior on slides, and more on whether community cancer centers embrace intensive retinal monitoring to leave hyperphosphatemia behind.
